Contact Form 7 – This is used everywhere because it’s simple to install, easy to use and to modify. It’s also made to integrate with the Simple Captcha plugin.

You don’t even have to create your form as it comes with a default contact form and a short code to drop into your webpage or widget. It really is that simple. And if you want something more complicated out of it, just tell your programmer or webmaster what fields you would like in your form and they can use this to set it up for you.
